What Are Peptides, and How Do They Work?

Peptides typically consist of 2 to 40 amino acids linked together in a specific sequence, and are claimed to have very specific, targeted action in your body. In functional medicine, therapeutic peptides are used to mimic or enhance the body’s natural signaling pathways.

Peptides are commonly used to:

What Patients Should Know

Here’s what many patients don’t realize:

  • The Federal Drug Administration (FDA) has only approved a select few peptides for specific indications—such as Semaglutide (Ozempic) for blood sugar regulation and Teriparatide for osteoporosis.
  • Peptides like BPC-157, TB-500, and Thymosin Beta-4 are often classified as research-only and cannot legally be compounded or sold for human use in the U.S.
  • Peptides from unregulated online sources often bypass FDA oversight by labeling products as “not for human consumption”, posing risks of contamination, incorrect dosing, or ingredient substitution.

Is Peptide Therapy Safe?

This is much debated among both allopathic and integrative medicine practitioners. The short answer is we don’t know and it very much depends on the peptide/drug in question among many other factors:

Established Peptides and Known Risk

For certain FDA-approved peptides, there is substantial clinical research supporting their safety, efficacy, and pharmacologic behavior in humans. These studies provide data on how the peptide is absorbed, distributed, metabolized, and excreted (pharmacokinetics), as well as its biological effects (pharmacodynamics). However, even with this level of understanding, well-documented side effects may still occur.

Lack of Data and Non-Approved Peptides

For peptides lacking robust clinical trials and human data, safety and efficacy cannot be assured. Without clear pharmacokinetic and pharmacodynamic profiles, it’s uncertain how these peptides are processed in the body—particularly in individuals with undiagnosed kidney or liver dysfunction. In such cases, impaired metabolism or clearance could lead to accumulation and potentially harmful effects.

Biologic Complexity and Unintended Effects

The human body is governed by countless biochemical interactions occurring at the cellular level every second. Our understanding of these processes remains incomplete. A peptide may be designed to target a specific pathway, but due to the complexity of cellular signaling, and genetic variability between individuals, it may unintentionally influence other pathways. This could result in unintended consequences, such as stimulating the growth of a cell population that may include precancerous or cancerous cells.

Genetic and Individual Variability

Peptides may not behave uniformly across all individuals. Genetic differences can influence how a peptide functions or which receptors it binds to. As a result, the desired therapeutic effect may not be achieved in every patient, and in some cases, unexpected or adverse outcomes could occur.
